Student Flu Mist and Staff Flu Shot Schedule
The Health Department will be coming to our schools for Staff flu shots and student flu mist as follows:

Wednesday, October 11th
10:30 AM to 11:15 AM, Albion Schools and Community
Thursday, October 12th
8:00 AM to 9:00 AM, Burley High School
9:30 AM to 10:30 AM, Declo Elementary
10:45 AM to 11:15 AM, Declo Jr. High
11:30 AM to 12:00 PM, Declo High School
1:00 PM to 3:30 PM, Raft River Schools & Community
Monday, October 16th
8:00 AM to 8:30 AM, Cassia High School
8:45 AM to 9:15 AM, Cassia Regional Technical Center
11:15 AM to 12:30 PM, Mountain View Elementary
12:45 PM to 1:30 PM, District Service Center
Tuesday, October 17th

Staff, please bring a copy of your insurance card otherwise you will get a bill for your vaccine. It should be covered under preventative care and they will bill insurance for you.
Parents will not need to be present for the vaccination. The flu mist is a nasal spray. We will need the paperwork below filled out and sent back to the school. Please send a copy of your insurance card otherwise you will get a bill for your vaccine.
